Registration is completed online (NEW STUDENT REGISTRATION) please fill out all of the registration forms (Links Signatures, Birth Verification, Proof of Residence, Health Information, Emergency Contacts, Legal Guardian Information, Student Registration, Permissions/Agreements and Home Language Survey) and submit them with copies of the following documents, birth certificate, proof of residence, health record, the student's last report card (if available) and current IEP (if applicable).
